Case Management is a resource department for patients, families, hospital staff and physicians. Nurses and social workers are available to answer insurance coverage questions, assist patients with development of a living will, address complaints and grievances, help patients arrange for discharge to home or any community placement setting, and provide referrals to a host of post-discharge services and programs that will ease a patient’s transition from the hospital setting.
Case Management is located in the hospital Support Service Building (Entrance “E”) behind the Health Information Management Department.
