Description:
Daylight: 7am – 3:30pm
Required: Bachelor’s degree – Master’s degree preferred / Additional certification in Community Health Work, Case Management or a related field is preferred / Experience in a healthcare or community-based setting is highly desirable / Local knowledge of community health & social welfare resources
Preferred: Strong interpersonal & communication skills, with the ability to build trust & rapport with diverse populations / Knowledge of community resources, social services & healthcare systems / Ability to manage multiple tasks & prioritize effectively / Proficiency in relevant technology, including electronic health records (EHRs) & Microsoft Office
